Enrollment Application

We are now accepting applications for the 2009 spring semester.

Step 1: Register

If you have not already done so, register online to save your spot in iQ Academy.

Step 2: Enroll

Once registered, fill out and print the Enrollment Application and submit to iQ Academy by fax, mail, or email.*

Required Documents

Submit the following required documents included in your Enrollment Application to complete your enrollment. These documents may be submitted separately:

Student Records Request
Alternative Learning Environment Statement of Understanding
Certificate of Immunization Status or Signed Statement of Exemption
Application for Attendance in Nonresident District (if student is NOT resident of Evergreen Public)

Step 3: Finalize Enrollment and Select Courses

To finalize your enrollment as quickly as possible please complete and return the following documents included in your Welcome Kit with your other application materials. Once your course selections are received in the office, they will be reviewed by an iQ Academy academic advisor.

Technology Use Agreement (Signature Page Only)
Copy of parent Drivers License (If using an iQ Academy computer)
Media Release Agreement
Student Profile (Optional)
Home Schooling Transcript (If applicable)
